UK Group recommendations for fish spa operators - The potential  transmission of infections, from fish-to-person (during the nibbling process), water-to-person , or person-to-person in fish spa settings have been studied.  Infection risk should increase if the client has an underlying health condition reducing effectiveness of their natural defences  against infection. Fish pedicures should not be intended for persons with eczema, psoriasis and diabetes.
